Player Registration Waiver

Before your player can join Aventura Premier FC, we need this signed on file. It only takes a few minutes:

1. Download the fillable form (PDF)
2. Open it in any PDF reader (works right on your phone) and fill in the player, medical, and parent/guardian fields.
3. Check the agreement box, then type your full legal name in the signature field — that serves as your electronic signature — and add the date.
4. Email the completed PDF to info@boldnationsports.com so we have it on file.
